The game's narrative follows Shimizu Hinako, an ordinary teenager whose life takes a dark turn when her town is enveloped in a mysterious fog, transforming into a nightmarish landscape. Hinako must navigate this unrecognizable environment, solve puzzles, battle bizarre foes, and make a crucial decision to survive. The story is described as "a tale about a beautiful yet terrifying choice."

Silent Hill f is set in the fictional town of Ebisugaoka, inspired by the real-life town of Kanayama in Gero, Gifu Prefecture, Japan. The developers meticulously recreated the town, capturing its intricate alleyways and using authentic reference materials and sounds to fit the 1960s setting.

Okamoto explained, "A noticeable feature of Japanese horror is the idea that terror can be found within beauty. When something becomes too immensely beautiful and perfect, it instead becomes deeply unsettling. Players will experience this world through the eyes of a young girl faced with a beautiful yet terrifying decision."

Okamoto emphasized that Silent Hill f is a standalone story, welcoming new players while offering Easter eggs for long-time fans. The game's writer, Ryukishi07, known for his psychological Japanese horror visual novels, brings his passion for the franchise to this new installment. Ryukishi07, a long-time fan of the series, aims to blend Silent Hill's origins with fresh challenges.

One significant challenge was creating a Silent Hill game without the series' iconic town. Ryukishi07 stated, "Naturally, from a creator’s standpoint, I feel I can confidently say that what we’ve made is a Silent Hill game. However, we’re interested in seeing how long-time fans of the series feel after playing, and if they agree."
